In the dystopian future, a worldwide plague creates a demand for organ transplants, leading a biotech company to introduce a controversial organ-financing program in the movie Repo! The Genetic Opera (2008). In this program, patients can obtain new organs, just like obtaining a car loan. However, there's a deadly catch - failure to pay back the loan results in brutal repossession.

Set against a dark, gothic backdrop, the film follows the lives of various characters affected by this organ-financing program. We meet Shilo Wallace, a young girl kept under strict house arrest due to a rare blood disease. Shilo's father, Nathan, is a respected surgeon and also secretly works as a repo man, responsible for repossessing organs from those who fail to make their loan payments.

As the story unfolds, Shilo discovers her father's secret life and becomes entangled in a dangerous web involving a flamboyant organ designer, a twisted family empire, and a rock star with a mysterious past.
